Fees and Insurance

We are committed to keeping our costs as reasonable as possible. Therefore, we ask for payment at the time of your visit in order to reduce costly billing.

• Insurance forms: Our staff will provide you with a claim form to file for insurance benefits.
  
  
• Surgical fees: Surgical fees will be discussed with you individually, so that you can plan accordingly for payment. Deferred billing plans can be arranged in cases of financial hardship; please discuss this with our staff before scheduling surgery.
